At Skyline Health Group, Van Nuys we offer FMCSA certified DOT Physicals at best price. Dr. Anthony Silva, DC at Skyline Health Group, Van Nuys is NRCME certified and registered to perform DOT physical examinations. The exams meet the requirements of the FMCSA Department of Transportation (DOT) and Department of Motor Vehicles (DMV) for commercial drivers.
What is DOT physical exam?
The DOT requires all commercial motor vehicle (CMV) drivers or those who are applying for Commercial Driver's License (CDL) to receive periodic physical examinations. The purpose of DOT physical examination is to ensure your own safety, as well as the safety of all the people who share the road with you. Hence during the physicals your overall health and physical condition is evaluated to ensure that you don’t have any health issues that would interfere with your ability to drive safely.
Only medical professionals who are listed on the FMCSA National Registry of Certified Medical Examiners can issue you a certificate. They have had the proper training in FMCSA regulations to determine if you’re fit enough to drive safely and fortify that you’re not wrongly disqualified.
A DOT medical card is valid for two years if the driver meets all the FMCSA standards. If you have a health condition that may inhibit your ability to drive, you may receive only a 1-year certificate. In certain conditions if your health issue is treatable, you may receive a 3-month certificate. After 3 months, if you show signs of improvement in medical condition, you’ll be eligible to get a 1 or 2-year certificate.

Before reading the cancellation policy please understand that these policies must be in place in order to schedule DRIVERS in a timely manner, as well as providing the ability to maintain a full book, and to protect DRIVERS from other ill patrons. These policies are not meant to punish or inconvenience, they are simply in place to protect all concerned.
What if I need to cancel or reschedule my appointment?
Your treatments are reserved especially for you. I value your business and ask that you respect the scheduling policies. All appointments cancelled without a minimum of 24 hour notice will be responsible for paying the full amount of the visit scheduled before any other service can be booked and even if no subsequent appointment is scheduled. When you book an appointment you are agreeing to these policies.
What if I’m sick?
Please do not come in for your physical if you are sick. This puts others at risk. If you arrive sick you will, most likely, be asked to reschedule your appointment. There is no charge if you are sick and cannot make your appointment. If you wake up sick please call or text right away so I may have time to fill the opening, please contact me before 9am if possible. I report to work based on my scheduled drivers.
What if I'm running late?
Usually up to 15 minutes late can be accommodated. This means in the examination room for the DOT physical exam to start, not walking in the door. More than 15 minutes late will need to be rescheduled. Your scheduled appointment time is taken very seriously and NEVER runs over into another DRIVERS scheduled appointment time. This is to avoid
inconveniencing other DRIVERS. If you think you may be more than 15 minutes late please call, as you may need to be rescheduled. This will save you a trip :-)
Common misconceptions cleared up
If I cancel but reschedule then it's all good, right?
Unfortunately this is not accurate, here's why. If you have an appointment cancelled at the last minute, or do not show up, I have lost an entire appointment of which I do not get paid, as well as a lost opportunity to fill that spot with someone else. To reschedule for another appointment you are taking 2 appointment slots but only paying for one. This is why you are charged for last minute cancellations. No-Shows and last minute cancellations are a factor in determining the price for your DOT PHYSICAL. Showing up, as scheduled, helps to keep prices from rising.
I can't help it if something comes up, I shouldn't have to pay you if you didn't give me a service.
This is why your appointments are always confirmed in advance. If a DRIVER doesn't show up I lose the opportunity to earn my pay and sit idle without pay until the next DRIVER is due to arrive. Time is wasted due to a late cancellation or no-show. Many services require pre-setup. No-shows or last minute cancellations are a loss of income, and time for the provider.
How do I pay for my missed appointment?
The card on your intake form will be charged. If your card has expired you will be asked to pay over the phone by credit card or ATM or sent a bill if all else fails.
